To book an appointment, please complete the new appointment form found here. We will be in touch with you once we review your application to schedule the appointment.

In the event that the provider decides that he or she is unable to take on your case for any reason, we will provide you with the contact information of another provider in the area.

What To Expect At Your Appointment

When you arrive for your first appointment, you will need to complete some paperwork. You will need to complete the intake form, adult or child history form, the consent to treatment form, the Health Insurance Portability and Accountability Act form, and your current medications list form prior to meeting the psychologist or therapist.

Sessions typically last 45 to 60 minutes, including the initial intake session. The first session will be devoted to gathering information about your or your child’s situation, and determining whether or not we feel a therapeutic alliance and effective treatment relationship can be established.  We also begin to formulate some initial lines of clinical inquiry and diagnostic formulation.  This process, leading up a conclusive diagnosis, may take up three to four sessions altogether.

We will bill your insurance company and will collect your co-pay/co-insurance at the time of your appointment.

If we determine at this initial assessment phase that we are not a right match for you, we will give you a referral to another provider.
